New Client Agreement

Reach out

Call, email, or send a note through our book an assessment page. Tell us a bit about your dog and what you're hoping to book.

Send records

We'll ask for current vaccination records — Rabies, Distemper (DHPP), and Bordetella — so we can meet your dog safely.

Book the evaluation

Every new dog joins us for a temperament evaluation to see whether or not your dog is a good match for the pack here at our facility.

2026 – 2027 Agreement

Client & pet agreement.

This agreement applies to current and future visits at Pet Bazaar NC, 1140 N Lake Park Blvd Unit C, Carolina Beach, NC 28428. Pet Bazaar NC agrees to provide care in a clean and safe environment.

All pets are boarded or otherwise cared for by us without liability on our part for loss or damage from disease, death, running away, theft, fire, injury to persons, other pets, or property by said pet, or other unavoidable causes, provided due diligence and care have been exercised. Dogs will not be taken off property unless given consent by the owner. Dogs will be able to interact freely with other visitor dogs unless otherwise instructed.

Vet-visit policy: all dogs must wait 10 days after vaccines (14 days if it's the first time receiving that vaccine). This keeps every dog at Pet Bazaar as healthy as possible.

How to submit

Please read before you send.

  1. Fill out every field and initial each of the seven policies below, then click “Generate PDF & email.”
  2. A formatted PDF of your completed agreement will download automatically to your device (check your Downloads folder — the file is named PetBazaarNC-Agreement-YourName.pdf).
  3. Your email app will then open with a message pre-addressed to PetBazaarNC@gmail.com.
  4. Important: the PDF is not attached automatically — browsers don't allow that for security. You must manually attach the downloaded PDF to the email (tap the paperclip / attach icon, choose the file from Downloads) before hitting send.
  5. Prefer to send it yourself? Just email the downloaded PDF to PetBazaarNC@gmail.comfrom any mail app.

  1. 1.When dropping my pet off they will NOT be sick/contagious for the past 30 days. I understand Pet Bazaar NC has the right to refuse my dog if they are showing signs of sickness, and if they do become sick they will be picked up right away.

  2. 2.My pet has NOT been to another dog daycare/kennel in the last 14 days. This is very important and a rule that cannot be broken. Just like taking human children to school/daycare, dogs are prone to illness and can be carriers of a virus. Violators will not be allowed back. Period.

  3. 3.Pet Bazaar NC requests you wait 10 days after all vaccines before returning to daycare/overnight care.

  4. 4.If Pet Bazaar NC reasonably believes your pet is having a serious health emergency, we will attempt to contact you at once, then your emergency contact, then use our best judgment. We will contact your designated veterinarian; if none, an available veterinarian. You agree that our reasonable decisions are in the best interests of your pet and agree to pay all veterinary charges promptly.

  5. 5.In consideration of the services rendered by Pet Bazaar NC, you release and waive any right to recover medical payments, property damages, or bodily injury damages for any liability due to ordinary negligence as a result of any injury that you or your dog may sustain from your own dog, any other dog(s), property, or any other cause on the premises. You assume the risk of allowing your dog to participate in daycare where they will interact with other dogs whose temperament is unknown.

  6. 6.If you bring a dog 6 months or older that is unspayed or intact, you or your emergency contact are responsible for them if they go into heat or need to be removed from the pack due to hormones.

  7. 7.If your dog remains in our care longer than the original dates reserved and you are unreachable for 72 hours, you give Pet Bazaar NC full custody of your animal and the right to re-home them to any home we deem acceptable.
